Instructions for your Supervisor

The Internship Supervisor's Evaluation form should be completed by the student’s immediate supervisor and discussed with the student prior to completion of the internship assignment. A copy of the form will be sent to the employer via email. He/She should print that email and give it to the student.

Should the employer prefer to use their company evaluation form, we have no problem with that. However, the form must still be submitted to Career Services and the student should receive a copy.
